Shutdown Risk: Why Compliance Isn’t Keeping People Safe

Confined space entry and working at height are still killing people. It doesn’t have to be this way. 

There are risks in heavy industry that everyone accepts as inevitable. Confined space entry is one of them. Working at height is another. They appear on every shutdown plan, every TAR schedule, every contractor briefing, normalised by repetition, managed by procedure, and accepted as just part of the job. 

But acceptance isn’t the same as inevitability. 

The data is unambiguous. Confined space entry and working at height consistently rank among the leading causes of serious injury and fatality across oil and gas, mining, wind, and nuclear. Not because operators don’t care. Not because procedures aren’t followed. But because the fundamental assumption, that a person needs to be in that space, at that height, to get the job done, is increasingly wrong. 

The shutdown paradox 

Shutdowns and turnarounds are where safety exposure peaks. More contractors on site. More simultaneous activities. More pressure to move fast and stay on schedule. And more confined space entries and working-at-height tasks than at any other point in the operational calendar. 

The paradox is this: the period when organisations are most focused on safety is also the period when they are most exposed to it. 

Peak demand drives shortcuts, not deliberate ones, but the kind that creep in when timelines compress and resources are stretched. A pre-entry gas test gets rushed. A permit takes longer than expected and someone makes a call in the field. A working-at-height task gets done with equipment that’s available rather than equipment that’s right. 

None of these are reckless decisions in isolation. In the context of a shutdown, under pressure, they are entirely predictable ones. 

What compliance gets wrong 

Regulatory frameworks across heavy industry have evolved significantly over the past decade. Permit-to-work systems, atmospheric monitoring, rescue standby requirements, fall arrest systems, the layers of protection are real, and they matter. 

But compliance and safety are not the same thing. 

Compliance defines the minimum acceptable standard for doing a dangerous task. It does not ask whether the task needs to be done by a person at all. And that is the question that the industry has been too slow to ask at scale. 

The technology to remove people from confined spaces and working-at-height environments exists. It is proven. It is deployed. It is running in operations right now across the same industries, the same asset types, the same shutdown contexts where people are still being asked to take on risk that technology could absorb. 

The gap is not capability. It is adoption.

A different risk conversation 

The traditional risk conversation in a shutdown goes like this: how do we make it safe enough for a person to do this task? 

The better question is: does a person need to do this task at all? 

Robotic inspection platforms can enter confined spaces, vessels, tanks, pipes, TLO bins, and return accurate ultrasonic thickness measurements, high-definition visual data, and 3D spatial models without a single human entry. Remote inspection systems can assess working-at-height assets from the ground, or from a controlled position, without putting anyone on a rope or a platform above a risk threshold. 

This is not a future capability. It is available now. And every unnecessary confined space entry, every avoidable working-at-height task that happens during a shutdown while this technology sits unused, is a risk that was chosen, even if the choice was made by inertia rather than intention. 

The cost of getting it wrong 

Safety incidents during shutdowns and turnarounds carry costs that extend far beyond the immediate human impact, though that alone should be sufficient reason to act. 

Investigations. Regulatory scrutiny. Reputational damage. Project delays measured in days or weeks. Insurance implications. And the long-term cultural impact on a workforce that witnessed what happened and draws its own conclusions about how much it is valued. 

The organisations that are getting this right are not doing so because they have more resources. They are doing so because they have changed the question they are asking at the planning stage, moving from “how do we manage the risk of this entry” to “how do we design the entry out of the process entirely.” 

That shift in thinking is available to every operator planning a shutdown right now.

Safety leadership in a TAR context is not about more procedures, more signage, or more toolbox talks. It is about using every available capability to reduce the number of moments where a person is placed in a position where something can go wrong.
